What is CDPAP?  Consumer Directed Personal Assistance Program.

How does CDPAP help? It allows those on Community Medicaid to have people they know and trust get paid for providing care as Personal Assistants (PA).

It important to note that unlike utilizing a homecare agency, it is the Consumer or Designated Representative (DR) who has the task of hiring, firing and training the Personal Assistants (PAs).

You may already know that there were some big changes in CDPAP a year ago. New York went from having about 700 CDPAP agencies to 1. That agency is called PPL. Last year I blogged here about these changes.

Here are 3 Tips if you are considering CDPAP or even if you have CDPAP already but are not yet registered

  1. Use a facilitator; they generally make the registration easier. You can find one near you using PPL link of facilitators—https://pplfirst.com/cdpap-https://pplfirst.com/cdpap-facilitators/
  2. The consumer must create an online account with PPL using 3 contacts—mobile phone, email address, and a 3rd contact (could be a relative or landline)
  3. Consumers and DR will be directed to: 
    1. Complete Profile – requires Medicaid CIN Number ( the 8 character number on the Medicaid care that starts with 2 letters and ends with a letter)
    2. Include the names of PA’s in the Association Section of the consumer’s profile
    3. Sign Memorandum of Understanding (MOU ) (in “Program Documents” in various languages)
    4. If DR—sign the Designated Representative form • Consumer and DR must register and have a PPL ID number before the PA can register.

 

Here are 4 Tips if you are looking to be a Personal Assistant (PA) for CDPAP.

  1. Each PA must create an online account with 3 contacts mobile phone, email address, and a 3rd contact. 
  2. Provide basic demographic information and relationship to the consumer. 
  3. Upload required documents (or fax, email, mail) (Download forms from PROGRAM DOCUMENTS – link on left side of home page ). 
    1. Offer Letter (consumer or DR must sign)
    2. Personal Assistant Agreement
    3. Designated Representative Form
    4. IRS Form W-4– State Form NY IT-2104-I– Payment Method Form
  4. Citizen or immigrant authorization to work
    1. Upload USCIS I-9 (with verification documents, US passport, green card, two other forms of ID (driver’s license, NYS ID, SSA card, etc.)
    2. After I-9 and other documents uploaded, PPL will schedule a ZOOM during which PA must show green card or passport. 
    3. Can be LONG wait time in queue for zoom (hours!)
